I understand Hay Creek Companies utilizes a drug-free workplace. Finally, I understand the successful completion of the pre-employment drug screening is in no way a contract for employment, expressed or implied.

CONSENT FOR PRE-EMPLOYMENT/EMPLOYMENT, RANDOM, OR REASONABLE SUSPICION DRUG/ALCOHOL TEST SCREEN

An offer of employment from Hay Creek Pallet, Inc. is contingent upon satisfactory completion of pre-employment testing for the presence of drugs, adulterants and /or alcohol. All applicants are required to complete this test. If an applicant does not complete this test they will not be considered for employment. Further, any job applicant whose test results are verified by the laboratory as positive or adulterated will not be eligible for employment.

I hereby CONSENT to allow the designated clinic to take a specimen of my urine and submit it for a pre-employment, random, or reasonable suspicion drug test screening. I FURTHER CONSENT to allow the laboratory testing service to make the results of such screen and other relevant information available to Hay Creek Pallet, Inc.

In addition, I understand that if hired, in the event of an accident or reasonable suspicion, I may be required to submit to a drug test during my employment. I may also be required to submit to random drug testing.

In consideration for such services being rendered on my behalf, I hereby RELEASE Hay Creek, Inc, the laboratory testing service, the collection facility, their respective officers, agents, and employees, from any and all claims which I might otherwise have due to such results being made so available. I hereby CONSENT NOT TO FILE ANY ACTION at law or in equity against Hay Creek Pallet, Inc. , the laboratory testing service, the collection facility, their respective officers, agents or employees in connection with the results of such screen being made so available, and I hereby agree to INDEMNIFY and SAVE HARMLESS Hay Creek, Inc., the laboratory testing service, the collection facility, their respective officers, agents, and employees from all damages, expenses, reasonable attorney's fees, and costs of court which they or any of them may suffer or incur, jointly or severally, due to the results of such screen being made so available.
